Diabetes Self-Management Education and Support: Facilitator Support Grant

Diabetes Self-Management Education and Support (DSMES) is the ongoing process facilitating the knowledge, skills, and ability necessary for prediabetes and diabetes self-care, as well as activities that assist a person in implementing and sustaining the behaviors needed to manage his or her condition on an ongoing basis, beyond or outside of formal self-management training. 

MPA has established a partnership with STRAND for the DSMES Facilitator Support grant. STRAND is a clinical pharmacy solution center. Their solutions are designed to help you implement and improve clinical services your pharmacy can offer. They cover all aspects of clinical care from chronic care management to clinical documentation to patient interventions and more. Their diabetes education solution is The Sweet Spot.

Grant funding reimburses for STRAND operations and administrative support to achieve AADE site accreditation. The STRAND support allows you to pursue site accreditation with greater ease. STRAND has a high level of experience, with nearly 200 pharmacy owners (500 if accounting for branch locations) nationwide using the STRAND support and its clinical platform to achieve AADE site accreditation successfully.
